Support coordination is designed to improve your ability to interact with and coordinate services in a complex social environment that includes informal, mainstream, and sponsored supports.
The support coordinator can assist you in achieving the following goals:
Start your plan.
Identify and address the barriers to using services, as well as the difficulty of doing so.
Identify support choices and connect you to the services you choose.
Assist you in determining how you can best utilize your funds.
Manage how the funds in your plan are used.
Establish service agreements with providers.
Engage with services and ensure that everything is working properly for you.
Manage and adjust your supports proactively in response to changing needs.
Manage any emergency situations that arise.
Increase your ability to participate in day-to-day activities.
Keep track of your spending and report the results to you/the NDIA.
