What Service We Provide

Extend Home Healthcare, an NDIS registered provider, will assist you in accessing NDIS core support services.

Services will differ depending on your specific needs, objectives, and funding categories.

Assistance with Daily Life

Helping you live your best life while maintaining as much independence as possible. Personal and domestic care, as well as assistance with home activities, will be provided.

Assistance with Social and Community Participation:

Supporting you to participate in community activities and other social activities that will help you develop skills and independence. Support for sport or art, TAFE, University, or other educational facilities, for example. Support can be given in a variety of settings in the community, such as one-on-one encounters or in a group setting in a center.

Health Consultation

Your specialized support coordinator can help you to: Identify and address any barriers to using your plan or participating with services. Begin working on your plan. Determine your options for support. Connect you to services. Assist you in understanding and managing your plan’s funding. Manage how you spend the money you set aside in your plan. Establish service agreements with providers. Begin your services and make sure everything is running smoothly. Keep track of your spending and report the results for you and the NDIA. Prepare for your evaluation.
