Support coordination is designed to improve your ability to interact with and coordinate services in a complex social environment that includes informal, mainstream, and sponsored supports.

The support coordinator can assist you in achieving the following goals:

  • Start your plan.
  • Identify and address the barriers to using services, as well as the difficulty of doing so.
  • Identify support choices and connect you to the services you choose.
  • Assist you in determining how you can best utilize your funds.
  • Manage how the funds in your plan are used.
  • Establish service agreements with providers.
  • Engage with services and ensure that everything is working properly for you.
  • Manage and adjust your supports proactively in response to changing needs.
  • Manage any emergency situations that arise.
  • Increase your ability to participate in day-to-day activities.
  • Keep track of your spending and report the results to you/the NDIA.
  • Prepare for your evaluation.
